(Miramichi) April 28, 2009 - Regional Health Authority B (RHA B) is an organization that is open, transparent and discloses information when an error has occurred including a breach of patient confidentiality. Recently such an error occurred related to two incidents of misplaced or missing patient files and information.
Public Health discovered three patient files were misplaced from the Fredericton/Upper River Valley area in December 2008. "An extensive search was conducted over the past several months before they were located," says Fonda Kazi, Vice President of Community and Primary Health Care. She says information contained in these files had personal information but limited health information as detailed patient information is stored electronically. All patients were notified on Monday April 27, 2009.
Cook Medical is an outside agency that develops health care devices and works closely with RHA B's Interventional Radiology programs at the Saint John Regional Hospital. "In early April the RHA received notification from the company that one of their staff members had a laptop stolen in January 2009," says Gary Foley, Vice President of Professional Services. "Three RHA B patients' names and birthdates were in the laptop. However, the laptop featured extensive security making it extremely unlikely the content could be accessed." All patients were notified on Monday April 27, 2009.
"In all cases patients have been provided reassurance and offered assistance including credit surveillance," says Kazi and Foley. The related protocols and procedures have been reviewed and enhanced. "As much as we hope these issues will never occur, we have taken the opportunity to improve our systems and educate our staff. It also brings awareness to our staff on the importance of coming forward with issues in a timely manner so we can assist them to make improvements."
